The shoulder is the most mobile joint in the body, which also makes it one of the most complex to manage. It relies on a precise balance of strength and control across the rotator cuff, the surrounding muscles and the structures that stabilise the joint. When that balance is off, the wrong things take the load.
For many people, the real problem isn’t the pain itself. It’s that the underlying cause was never properly identified.
Rest reduces the irritation temporarily, but the same movement patterns and weaknesses are still there when activity resumes. Without addressing what’s actually driving the problem, the pain returns.

Shoulder pain isn’t one thing, and the right treatment depends on what’s actually going on. Here are the most common conditions our Cairns physios assess and treat.
The rotator cuff is a group of four muscles that control and stabilise the shoulder joint. Tears can occur suddenly from a fall or sporting incident, or develop gradually through repetitive load and poor movement patterns. Tendinopathy, where the tendon becomes irritated and overloaded without tearing, is even more common. Both respond well to targeted shoulder physio that addresses the load going through the tendon and builds the strength to support it.
For post-surgical rotator cuff cases, we provide structured rehab from the early protective stage through to full return to sport or function.
The bursa is a small fluid-filled sac that cushions the structures inside the shoulder. When it becomes inflamed, it causes pain with overhead movements and often at rest. Shoulder bursitis is frequently a secondary problem, driven by a movement or strength issue rather than being the root cause. Treating the bursa alone without addressing the underlying driver is why many people find it keeps flaring back up.
Impingement occurs when the tendons and bursa in the subacromial space get pinched with certain movements, typically overhead reaching or lifting. It’s one of the most common causes of shoulder pain we see and, in the majority of cases, responds well to physio without the need for injections or surgery.
Frozen shoulder is a condition where the capsule surrounding the shoulder joint thickens and tightens, significantly limiting movement. It progresses through three stages (freezing, frozen and thawing) and can take 12 to 24 months to fully resolve without treatment. Physiotherapy at the right stage can reduce pain, maintain available movement and significantly shorten the recovery timeline.
Shoulder instability, where the joint feels like it might slip or give way, is common in younger athletes and people who have previously dislocated their shoulder. Labrum tears often cause instability and a sense of the shoulder not sitting right. Many cases can be managed effectively with rehabilitation focused on strength and joint control.

Physio can fix many shoulder problems, not just manage them. The key is identifying the root cause. When the underlying issue is properly assessed and treated with a structured plan, most patients see lasting improvement. For more complex cases like a large rotator cuff tear, physio may be part of a broader plan that includes surgical opinion, but it is nearly always the recommended first step.
Not necessarily. Research consistently shows that many rotator cuff tears, including partial and even some full-thickness tears, respond well to physiotherapy without surgery. The decision depends on the size of the tear, your age, your activity level and your goals. We’ll give you an honest assessment and, if surgical opinion is warranted, refer you to the right specialist.
It depends on the condition. Simple impingement or bursitis often improves significantly within 6 to 8 weeks of consistent treatment. Rotator cuff tendinopathy typically takes 3 to 6 months. Post-surgical rehab and frozen shoulder recovery can take longer. Your physio will give you a realistic timeline based on your specific presentation at your first appointment.
Yes, though the approach changes depending on which stage you’re in. During the freezing stage, the focus is on pain management and maintaining available movement. In the frozen stage, gentle mobilisation and targeted exercise help maintain function. In the thawing stage, progressive strengthening and range of motion work speeds up recovery. Starting physio early can significantly reduce the overall duration.
No, you can book directly with us without a referral. If you have a GP Management Plan, a WorkCover claim, or DVA cover, relevant documentation will be needed to access those funding pathways. Give us a call if you’re unsure what to bring.
Private health insurance with extras cover typically includes physiotherapy. Medicare cover is available if your GP refers you under a Chronic Condition Management Plan, which allows a set number of Medicare-rebated sessions per calendar year. We also accept WorkCover Queensland, DVA, NDIS and motor vehicle accident (MVA) claims. Fees are listed in the online booking system.
